Special Dietary Information

Smyrna School District makes reasonable accommodations for special dietary modifications for all students. Parents of students with food allergies must contact the school nurse and fill out the “Request for Diet Modification Annual Medical Statement”. This form must be filled out and signed by a state licensed health care professional, and returned to the school nurse.

Pending appropriate signed documentation, the below accommodations are made.

Dairy Allergy

All cheese, yogurt and other items containing milk, whey, or casein will be eliminated and substituted with an alternate source of protein.

Lactose Intolerant

Lactaid and Soy Milk are available for students who fill out the “Parent Allergy Letter”and return to school cafeteria, school nurse or Nutrition Services office.

Gluten/Wheat

All wheat/rye/barley will be eliminated and substituted appropriately.

Soy

All items containing a soy protein, soybean oil or soy lecithin will be eliminated and substituted appropriately.

Egg

All items containing egg/egg white/egg yolk will be eliminated and substituted appropriately.

*Unless a state licensed health care professional specifies otherwise on the “Request for Diet Modification Annual Medical Statement”.

Peanuts and Tree Nuts

The only item Nutrition Services serves with peanuts are sealed Uncrustable sandwiches. These will be eliminated and substituted appropriately.

Fish and Shell Fish

All items containing shellfish or fish, depending on student allergy, will be eliminated and substituted appropriately.

Fish sandwiches and fish sticks that are offered are made fom 100% Pollock.

Sesame

All items containing sesame will be eliminated and substituted appropriately.

Other Uncommon Food allergies:

 – Fruits (Peaches, Pears, strawberries, tomatoes, etc.)

– Vegetables (Peas, beans, broccoli, corn, etc.)
